If your child is covered under PEEHIP Insurance and you want them to receive a Flu shot at their school:
- See the schedule below for when their school will have their ADPH Wellness Clinic.

Complete a flu vaccine form (click here for PEEHIP Flu Form) and submit it to the school nurse at your child’s school. (Please sign the form, even though it will let you electronically sign, the clinic will not accept electronic signatures).
Oct 1 - Transportation from 7:30 am to 10 am and Sanford Middle from 11 am to 2 pm
Oct 6 – Dean Rd from 8 am to 11 am and BOE from 12 pm to 3 pm
Oct 7 – Woodland Pines from 8 am to 11 am and Yarbrough from 12 pm to 3 pm
Oct 8 – Richland from 8 am to 11 am and Creekside Elem from 12 pm to 3 pm
Oct 13 – Wrights Mill from 8 am to 11 am and Auburn Early Ed from 12 pm to 3 pm
Oct 14 – Auburn Jr. High from 8 am to 2 pm
Oct 15 – Drake from 8 am to 11 am and East Samford from 12 pm to 3 pm
Oct 20 – Auburn High from 7:30 am to 3:3 0 pm
Oct 27 – Pick Elem from 8 am to 11 am and Cary Woods from 12 pm to 3 pm
